Learning Tree Childrens Academy

During the inspection a caregiver was observed to be alone with a group of children when the conditions of their background check stated that they cannot be alone with a child or group of children.

Transportation training for the director is not current.

An individual that had a prior eligible background check was inactivated on 09/15/2025 and has continued to be present at the operation without an eligible background check at the operation.

It was found during the investigation that a caregiver roughly grabbed up a child by their arm and placed them in timeout. Additionally, after the child received an injury the caregiver grabbed the child by their arm and pulled them to another area.

It was found during investigation that a caregiver inappropriately disciplined a child by placing the child in the bathroom area and closing the door. While the child was in the bathroom the child sustained an injury and needed medical attention.

2 out of 10 records reviewed was missing current First Aid/CPR training. 1 out of 10 records reviewed was missing 24 out of 24 required annual training hours for training year 8/2023-8/2024.

2 out 10 records reviewed was missing a current immunization in their file. 2 out of 10 records reviewed was missing an immunization record in their file.

1 out of 10 records reviewed did not have a Notarized Affadavit in the employees file.

It was found during the inspection that 2 children that required a Food Allergy Emergency Plan did not have doctors signatures on the plan.

It was observed in a classroom that the diaper changing mat material was cracked and missing in areas and the foam in the mat was exposed.

It was found during the investigation that a child was left unsupervised on the playground for approximately 5 minutes.

It was found during the inspection that a slide on the south playground was cracked and posed a pinching hazard. This was corrected at the inspection by putting tape over the cracks until they can replace the slide.

It was found during the investigation that a child in care had access to and swallowed an object that is considered a choking hazard.

Based on the evidence obtained a child in care was pinched by a teacher, causing bruises.

1 out of 10 records reviewed did not have a Health Statement and has been attending the daycare for over a year.
